Sean,
Please send more information.  For starters, a log file would be useful,
also some more of the standard output for context.
The message you show is generated by Eos_wrapped.  The following 
information would be helpful:
- From where is this Eos_wrapped call made?
  Is this the very first such call?
- On what block is this call operating?
  Is it the very first block?  What is the refinement level; Is it a leaf 
  block or a parent block or neither?
Btw. was this a parallel run?  If yes, does the same problem happen on one 
processor?
