Rodrigo,
I am attaching the log file of a similar simulation with the same setup.
With regards to your questions:
1) I am not using gravity.
2) I am using Split PPM. I was originally using unsplit when using cartesian coordinates, but it didn't seem to work right in spherical, so I switched to split.
3) I was not aware of the "hybrid_riemann" flag. Looking at the log files, I see that it is currently on the default value of false. I will see if turning it on helps.
4) At the initial resolution, which only applies for the first few seconds of simulated time, the resolution is around 1.0E+10 cm. Over the course of the simulation, the maximum level of refinement drops from 10 levels to 5.
5) I am not certain at this moment of the Mach number of the flow, but I will look into using the "eint_switch" flag.

Hi Michiel,
You could be having problems for a number of reasons. Some questions that might help narrow things down:
1) Are you using gravity? This looks like a stellar envelope.
2) Which hydro solver are you using (split PPM, unsplit, or other)?
3) If using the split PPM solver, are you using the "hybrid_riemann" flag to deal with the odd-even decoupling instability (due to shocks aligned with the grid)?
4) Regarding the inner regions, how many cells per decade in radius are you using (i.e., how well resolved is the density gradient)?
5) What is the largest Mach number of your flow? If it is too large, you might want to use the "eint_switch" flag to evolve the internal energy instead of total energy when the Mach number exceeds a threshold value.
